- FCPS Part II training: MBBS with FCPS Part One
- MCPS training: MBBS with house job and one year clinical experience
- Valid registration with Pakistan Medical and Dental Council
- Domicile / PRC: Karachi
- Two recent photographs
- Copy of CNIC
