Series: Understanding MSDS for Dialysis Facilities | Post 1 of 40

###### Eight Machines and the Wrong File

Dr. Kamran Shafiq, a nephrologist, opened a standalone dialysis centre on High Street, Sahiwal, with eight hemodialysis machines, a reverse osmosis plant and two shifts a day. Most of his patients had end stage kidney disease and travelled two or three times a week from villages around Chichawatni and Harappa.

When the centre began preparing for the Punjab Healthcare Commission (PHC), his manager, Mr. Naeem Akhtar, borrowed the MSDS file of a 40 bed hospital and started copying its checklists on wards, the labour room and the operation theatre.

Dr. Kamran closed the file. “We have no labour room and no theatre. What we have is water, machines, needles and blood, for the same patients, for years. The PHC has written a separate manual for exactly that.”

###### Why Dialysis Has Its Own Standards

The Punjab Healthcare Commission Act 2010 established the PHC to improve the delivery of healthcare services and to ban quackery in Punjab. The Commission regulates all healthcare establishments (HCEs), public and private, through registration, by prescribing and enforcing category specific service delivery standards, and by issuing a licence subject to compliance with them.

The MSDS for Category I and Category II hospitals already include the quality aspects of dialysis provided within those hospitals. Dialysis facilities working independently needed standards of their own. The manual states that it covers standalone dialysis centres and may also be applied to dialysis units functioning as an integral part of Category I and Category II hospitals, in both the public and private sectors.

The draft was prepared by the PHC team with nephrologists, urologists, physicians, senior dialysis technologists and managers, including a province wide consultative workshop on 28 February 2017. It was scrutinised by the Technical Advisory Committee on 29 December 2020, approved by the PHC Board of Commissioners, reviewed by an expert committee of the Specialized Healthcare and Medical Education Department, approved by the Cabinet and notified by the Government. The manual notes that it was finalised with ground realities in view, so that small centres serving marginalised communities can also implement it.

The document in use is the MSDS for Dialysis Facilities (Hospital Units / Standalone Centres), CG-14MS-Ed1, November 2023.

###### What the Manual Contains

The manual has 37 standards in 10 functional areas:

- Responsibilities of Management (ROM): Standards 1 to 4
- Facility Management and Safety (FMS): Standards 5 to 12
- Human Resource Management (HRM): Standards 13 and 14
- Information Management System (IMS): Standard 15
- Quality Assurance (QA): Standards 16 to 18
- Access, Assessment and Continuity of Care (AAC): Standards 19 and 20
- Care of Patients (COP): Standards 21 and 22
- Management of Medication / Procedure (MOM): Standards 23 to 25
- Patient Rights and Education (PRE): Standards 26 to 29
- Infection Control (IC): Standards 30 to 37

Each standard is a single statement followed by its compliance requirements and a survey and scoring rule. Most standards also carry guidelines, with formats, tables and frequencies that show how the requirements are to be met.

###### The Annexures

- Annex A: maximum allowable levels of contaminants in dialysis water and the composition of a standard hemodialysis solution
- Annex B: RO water treatment flow chart
- Annex C1 to C5: job descriptions of the nephrologist, MO/WMO, dialysis technologist, charge nurse and dialysis technician
- Annex D: classification and segregation of clinical waste
- Annex E: calculation of PPE and materials
- Annex F: instrument sterilisation flow
- Annex G: infection prevention and control precautions for all patients
- Annex H: training of staff on infection prevention and control in dialysis
- Annex I: training of patients and family members

Annexes J to P record the experts, reviewers and notifications.

###### Where the Licence Fits

The manual describes the MSDS as the basic framework for assuring quality and patient safety in dialysis services. Compliance with the standards makes a facility eligible for a PHC licence and for lawful operation. Even the signboard of a standalone centre is expected to carry its PHC registration or licence number.

###### What Dr. Kamran Did Next

The hospital file went back. Dr. Kamran divided the 10 areas among his team. The medical officer, Dr. Rizwana Noreen, took patient records, long term monitoring and consent. The senior dialysis technologist, Mr. Asim Iqbal, took the machines, water treatment and maintenance. The charge nurse, Ms. Farzana Yousaf, took infection control and waste. Mr. Naeem kept management, staffing and patient rights.

Each owner read the compliance requirements line by line. Within a month, every standard had a gap list, a responsible person and a target date.

###### A Quick Self-Check

- Do you hold the MSDS for Dialysis Facilities, CG-14MS-Ed1, November 2023?
- Is your facility a standalone centre or a unit integral to a hospital?
- Can your senior staff name the 10 functional areas and the standards in each?
- Has each of the 37 standards been assigned to a named person?
- Have you read the annexures that give formats, flow charts and job descriptions?
